The HIM Department is the main source for your medical records and protected health information. Services provided in our area are release of information, completion of birth and death records and patient portal.

Your Medical Information

MRHC is happy to provide you or your provider with your medical records and images. Specific forms are necessary for any release of information as required by MRHC policy and federal HIPAA regulations. You will need to fill out an Authorization for Release of Medical Information form – PDF (release form) and bring it with you to request your records along with a photo ID, or see below for other ways to request your records. Requestor must be 18.

Providers Requesting/Sending Records

  • Please email/fax/mail your request on office letterhead with pertinent patient and requesting provider information.
  • We are able to share images electronically through the Nuance Powershare Network. Powershare allows us to send imaging studies much quicker and more secure than burning CDs. Images can be sent to any organization that is also a part of the Powershare Network.

Where the HIM Department is Located

The Health Information Management Department of the hospital is located on the first floor of the hospital and can be easily accessed by entering through Entrance A. Just follow with signs for Medical Records. The department is open from 8:00 a.m. to 5:00 p.m., Monday through Friday. Personnel are on call on weekends and for emergency situations.

Your Medical Records Online at MyHealth.mrhc.org

MyHealth, provided by Magnolia Regional Health Center, is a secure private web portal that allows you to access your health records online.  Updating your personal information, checking your visit history, and viewing your scheduled appointments is all available in one place any time of the day and night.

Our patient portal has a convenient self-enrollment tool that allows patients to sign up using an email address and demographic information. An email address must be on file with MRHC in order to sign up.

Health Information Exchange

Magnolia Regional Health Center and Magnolia Regional Health Center owned clinics (MRHC) understands that you may receive some of your health care from providers other than MRHC. We also know that it is important for those providers to have up-to-date information about the health care you received at MRHC so they can provide you with the best care possible. Therefore, for the benefit of our patients, MRHC participates in health information exchanges (HIEs) to facilitate the secure electronic sharing of your health information with other HIE participants involved in your health care. Your medical information will be automatically made available by MRHC through HIEs unless you choose to opt-out. If you choose to opt-out, MRHC will not share any of your health information through the HIEs and your non-MRHC health care providers will not be able to access your MRHC healthcare information through the HIEs. You may opt-out at any time. To do so, please complete our HIE Opt Out Form and submit it as indicated on the form. If you opt-out and later change your mind you may opt back in at any time. To do so, please complete our Cancellation of HIE Opt Out Form and submit it as indicated on the form. Please understand that if you opt-out of the HIEs that MRHC participates in, MRHC will still be able to access your health information in the HIE that another participant has shared.
